Native People of Thunder Bay Development Corporation

Instructions

You may obtain an application form by contacting our office at (807) 343-9401 or by fax at (807) 345-1075, or you may print out a copy of the application below. If you have any difficulty completing this application, please contact the Native People of Thunder Bay Development Corporation office.

  1. Please complete all relevant sections that pertain to your application (family or senior) and forward to the address above.  Please print all information in ink.
  2. Before completing the income and assets sections of this form, please read the definition of income.
  3. Any additional forms mentioned in this housing application are available on our website. 

The housing program has only single-family dwellings and does not lease to single people. (Note: Family can be defined as “single people with one or more children”). Single people age 55 or older, are also eligible for housing.

You will be required to complete a consent form. It is very important that you read the information about the consent form.

The following documentation must accompany your application for it to be processed: (Please note that your spouse/partner will also have to include their information as well.)

1. INCOME VERIFICATION
You must include your most recent pay stub, we may ask for past pay stubs as well.

2. INCOME TAX RETURN SUMMARY
Income tax return summaries must be from the year prior, or after the current tax year.

(NOTE:  T 1’s, T 4’s, and Assessments will not be accepted).

 

3. COPY OF ALL STATUS, METIS, INUIT VERIFICATION
You must provide you and your family’s aboriginal, Metis, or Inuit status cards.  A housing unit shall be leased to persons of Aboriginal ancestry.  If the unit is leased to Tenants, at least half (50%) of the occupants are to be of Aboriginal ancestry.  (Note: Persons are of “Native ancestry” if they are Indians as defined in the INDIAN ACT (CANADA), persons commonly referred to as Non-Status Indians, Metis, or persons of Inuit race).

If you are faxing your application back to our office, please note that photocopies of status cards are very dark and/or are not legible, and may need to be mailed or hand-delivered to our office.

 

4. RESIDENTIAL HISTORY & LATEST RENT RECEIPT
You must provide landlord history for the past two years, and a copy of your latest rent receipt(s).

5. MEDICAL DOCUMENTATION
If you are applying for low income housing due to medical reasons (e.g. kidney dialysis, etc.), please have your doctor fill out the Attending Physician’s Report, and return with your application, so we may better accommodate your needs.  To print out a copy of the Attending Physician’s Report, please click here.

6. CUSTODY VERIFICATION
You will be required to include custody papers with your application.
